If it's not too hard to get to, pull it off and check it out. I went through 3 in 6 yrs.
I had a waterpump take out 2 and an oil seal fail, taking out the 3rd.

Moisture and/or oil will make its way to the bottom of the opti when it sits. So when you fire it up, it will miss on 1 or more cylinders. As it runs, the vacuum on the opti will pull the oil/moisture out into the intake. That's why it eventually clears up. I never got a misfire code.
This is just from my experience, and might not be your issue. I'd replace it with a GM performance opti. $$$$$$
 
Just a thought but have you checked the plugs to see if you possibly have a couple oil fouled? Sometimes we overlook the simplest **** first.

I don't blame you. Fwiw, when I saw my LT-1 drive away for the last time I had tears of joy in my eyes.
 
I got lucky and my LT1 misfire was just a bad plug at RBD this year.
However a few years back the opti went bad due to the previous owner leaving the vent lines off. Best I can remember it would idle fine but would pop and miss when hard in the throttle. I replaced it with a new GM one that has the vacuum. When installing I completely sealed it with silicon. It has since been completely under water once, normal muddy trail riding and pressure washed the last 3 or 4 years with zero issues. Has been a great junkyard engine and I have a lot of confidence in the sealed GM opti. Just my experience.
